Qlarant worked with the California Department of Developmental Services (DDS) and the Department of Public Health (DPH), performing an Independent Consultative Review Expert (ICRE) role at the certain Developmental Centers. Due to the decertification of several Intermediate Care Facility (ICF) units, Qlarant was contracted:
- to conduct a Root Cause Analysis focusing on the issues and concerns identified by licensure,
- to develop a facility action plan, and
- to monitor the effectiveness of the action plan for licensure compliance, assessing and evaluating the ‘real-time’ progress being made in meeting the outcome measures, improved delivery of care, and improvement in facility and resident quality of care.
Monthly, the onsite monitor and other select team members reviewed specific milestones in the action plan and provided recommendations and technical assistance to support quality improvement efforts. Reports were generated monthly and reviewed with the developmental center, DDS, and DPH. Qlarant also provided onsite monitoring at one Developmental Center as that facility closed. Monitoring continued until no further residents remained at the facility.
