Why It Is Important to Participate

NCI Surveys Call Out National Core Indicators® Surveys collect information from the person receiving services or their family about the quality of services and supports being provided from their perspective.  These surveys ensure people with lived experience can participate in helping shape the quality of service delivery systems at state and federal levels.

How Surveys are Conducted

The person receiving services, as well as their supports could each have a role in conducting the NCI® survey.

Role of Each Participant

The person receiving services determines if they would like to participate in the in-person survey. If the person has a court appointed legal guardian, the legal guardian may also be involved in the decision. The person will decide when and where the survey will take place and how they want to participate. If participating in the NCI® In-Person Survey, the person can decide whether they want to participate face-to-face or if they want to do it remotely using a video conference. If participating in the NCI® Aging and Disability survey, the person can also decide if they want the survey to be conducted via telephone.

The family can participate in multiple ways. If they receive a family survey, they can fill it out and mail it back in or complete it online. If they have a family member who is participating in either the In-Person Survey or the Aging and Disability survey, they may help support their family member to complete the survey by acting as a proxy, but only if the person needs support during the survey.

The provider who renders services may be contacted by the Qlarant NCI® Surveyor to help set up the survey for a person who has agreed to participate. The provider can help explain the NCI® survey to the person and what it means to participate. If the person wants to complete the survey remotely, the provider may be asked to help the person get connected to the video call. As necessary, the Qlarant NCI® Surveyor may ask the provider to provide or validate information needed to complete the background section of the survey, which could include such information as demographic, health, or employment information. The provider may also be asked by their state to complete the online State of the Workforce Survey.

The service or support coordinator/case manager may be contacted by the Qlarant NCI® Surveyor to obtain current contact information and/or help set up the survey with the person or family. They may be asked questions by the person or family member about the survey, the process, and value in participating. As necessary, the Qlarant NCI® Surveyor may ask the service or support coordinator/case manager to provide or validate information needed to complete the background section of the survey, which could include such information as demographic, health, or employment information.

  • For adults with intellectual and developmental disabilities and their families
  • Can be conducted face-to-face or video conference
  • For family members and/or legal guardians to complete
  • Completed by mailing the survey or direct entry online using a weblink
  • There are 3 survey types: Adult Family Survey (AFS), Family Guardian Survey (FGS), and Child Family Survey (CFS)
  • For provider agencies who render either services for the intellectual and developmental disabilities or aging and disability populations
  • This online survey collects comprehensive data on the states direct support professional workforce to examine workforce challenges, identify areas for further investigation, and benchmark their workforce data.
  • This survey is completed by provider agencies who report on wages, benefits, and turnover of the direct care professional workforce, hired by their agency
  • For the aging or disability populations to complete
  • This survey can be conducted most commonly by telephone but can be done either face-to-face or by video conference
